Medical Record
Knowing the details of your family’s health information can come in handy in the event of an emergency. Click on the top right link: “Download printable booklet: Family Health and Medical Record.” Print out this 18-page form and fill in the necessary information. Make several copies: keep one at work, at school, and a few at home. Print extra copies of pages 7-12 if you have more than one child. Be sure to update the information regularly. The Texas Cooperative Extension is an educational organization provided by the U.S. government, the state government through Texas A&M University, and your county government.
